If you haven’t already received your vaccine, Aliton’s Pharmacy Home Healthcare Centers is now offering COVID-19 vaccinations at our convenient downtown Port Jervis location to New York State residents or anyone working or enrolled in a NYS school who meet the current eligibility requirements.

STEP 1: BE SURE YOU ARE QUALIFIED
• Are you a NY State Resident?
• Are you employed in NY State?
• Are you enrolled in school in NY State?

BE SURE TO BRING WITH YOU ANY OF THE FOLLOWING DOCUMENTS TO SHOW ELIGIBILITY
NY State requires proof of NYS residency, employment in NY state, or current enrollment in a school located in the Sate of New York.

NY STATE RESIDENT
Please present one of the following:

  • Government issued ID
  • Statement from a landlord
  • Current rent/lease/mortgage documents
  • Current mail or school records

EMPLOYEED IN NEW YORK STATE
Please present one of the following:

  • Employee ID card or badge
  • Letter from your employer
  • Pay stub

NYS ENROLLED STUDENT
Please present one of the following:

  • School ID
  • Current school transcript

Under 18 years old must have a parent or legal guardian consent
